Quality of care is of utmost importance to us, as evidenced by the fact that we are Joint Commission accredited.The Registered Nurse Case Manager will work with our Enterprise Director of Nursing and Janesville Branch Manager to oversee the care provided to our clients. A typical day may include performing new client assessments, overseeing the care of existing clients (personal care and skilled), and care coordination with skilled service organizations and hospices. Compassion, patience and an outgoing personality are qualities that will build trust and a level of comfort with clients and their families. The Case Manager will work primarily (approximately 75%) in the field and 25% of the time in the office.Case Manager Essential Duties and Responsibilities:Provide care as defined within the scope of the state's nurse practice actMake the initial evaluation visit and regularly reevaluate the client's nursing and/or personal assistance needsAssist with the intake of new clients and inform potential clients of our services and differentiatorsProvide those services requiring substantial and specialized nursing skillInitiate appropriate preventive and rehabilitative nursing procedures and counsel client and family on nursing related needsPrepare clinical and progress notes and summaries of careAssist in the development of organizational goals and agency performance improvement activitiesAdminister agency policies and proceduresAssist with assessing the competency of staff to promote quality, continuity and safety of client careProvide information to be used for statistics, reports and records for purposes of evaluation and reporting of organizational activitiesAssure proper maintenance of clinical records in compliance with local, state and federal lawsPromote home care referrals in the health care communityEducation and Experience:Registered nurse with current unencumbered license to practice professional nursing in WisconsinMinimum of three (3) years of documented experience required.
