  • *This position will be supporting members in Janesville and the surrounding areas within 2 hours driving distance.**


    As a Casual RN Care Manager, you will be responsible for partnering with your fellow colleagues to provide top quality care to our members. Our members, frail elders and those with disabilities, are looking to you in providing assistance when it comes to their

    health and wellness. Some responsibilities will include the following:



    • Take lead on health issues for the members.
    • Identify and coordinate clinical and functional supports needed for the member's health and safety.
    • Participate in the development and ongoing review of member's care plan.

      The Casual RN Care Manager supports our S.O.S (Support Our Staff) Program and works limited hours (up to 28 hours/week maximum) and generally, up to three days per week covering extended leaves and/or vacancies. Casual staff are assigned to the S.O.S. Program

      Coordinator who manages assignment needs on a monthly basis. This position is given the opportunity on an ongoing basis to accept or deny a work request for the upcoming month. Hours are not guaranteed, and the schedule will vary week to week. Travel within

      a specified region, may up to two hours, is required per business need.

        Requirements Qualified applicants must have an Associate or Bachelor's degree from an accredited school of nursing and current Wisconsin Registered Nurse license. Care management experience and/or one (1) year of nursing experience within the last five (5) years is

        preferred. Must have a valid driver's license.
