Job Summary
Seeking a skilled and compassionate Registered Nurse to provide high-quality patient care in a home health and hospice setting.

Job Description
Coordinates and facilitates interdisciplinary plans of care for patients in their homes, community-based facilities, and other settings. Performs comprehensive medical assessments, develops and implements care plans, and provides skilled nursing care to patients with complex medical needs.

Key Responsibilities

• 
  
• Assesses patient needs and develops individualized care plans in collaboration with the healthcare team.
  
• Provides skilled nursing care, including wound care, IV therapy, and medication management.
  
• Collaborates with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  
• Documents patient information, including medical history, treatment plans, and progress notes.
  
• Participates in quality improvement initiatives to enhance patient care and safety.
  
  
  Requirements
  
  • 
    
  • Graduate of an accredited nursing program.
    
  • Current licensure as a Registered Nurse in the state of employment.
    
  • Basic Life Support (BLS) certification.
    
  • Valid driver's license.
