Have: RN License for WI • Wound Care Experience • Certified CPR/BLS
• Valid DL Reliable Vehicle Valid Car Insurance • Mileage
Reimbursement: .53 cent per mile • Counties: Fond Du LAC AND Dodge,
Columbia counties • Home Care Home Base-HCHB Highly Desired if they
don't have this experience Matrix Care is acceptable or any
Electronic Medical Record • Additional Locations in Jackson,
Waterford, and Sheboygan Serving Wisconsin Since 2007

that's submitted does not have experience with HCHB or Hospice, the
team is required to train them. • For training time the bill rate
will be reduced to 60 an hour and increase to the pull rate in the
tool after they've completed training. They would need to talk to
the candidate to assess the number of training hours/days needed. •
For someone who has Hospice but no HCHB experience would probably
need 16 hours of HCHB training. • For a candidate with no Hospice
experience it would vary based on experience with: Working with
frail elderly Working with patients in a Home setting Working with
/in Skilled Nursing (Nursing Homes) Assisted Living Facilities • As
well as any of the following preferred experience: Case Management
- oversight/collaboration with other members of the Hospice team. •
( CNA, Social Worker, Chaplain, Music Therapist) Care Planning
Documentation - narrative assessment of visit/patient status
Coordination of care with Families, Facility staff and families
Knowledge of End of Life Medications Comfort in education of
patient, family and staff of end of life signs and symptoms,
disease processes and symptom management. • Wound Care • IDG
Documentation/ Recertification Pain Management Skills competency
checklist-REQUIRED and must be uploaded during compliance checks •
Elder Abuse training • HIPAA training • Infection prevention and
control • OSHA compliance • Education/skills training received in
the past 12 months. This includes copies of any training they may
have received in the past 12 months. Primary Responsibilities:
Perform comprehensive assessment of high risk members. Collaborate
with primary care providers to ensure the implementation of an
individualized, comprehensive care plan with specific interventions
designed to engage the member. Address, identify, and continuously
reassess cost-efficient, appropriate levels of care. Put non
medical support in place to ensure compliance with treatment plans
(such as housing and transportation). Coordinate care transitions
as needed. Coordinate with physicians, social workers, discharge
planners, and claims professionals in transitioning patient to
appropriate level of care. Engage specialty resources as needed to
achieve optimal resolution. Keep detailed records of clinical,
functional, and fiscal outcomes during the management process.
Identify opportunities for health promotion and illness prevention.
Prevent adverse patient occurrences when possible and intervene
quickly if prevention is not possible, thereby minimizing poor
outcomes.Act as patient advocate by protecting privacy and
confidentiality issues. Qualifications: Associates or Bachelor?s
Degree in Nursing or related field. Current, unrestricted state
Registered Nurse license. Ability to analyze complex medical
