Home Health Physical Therapist for a travel job in Waukesha,
Wisconsin. Job Description & Requirements - Specialty: Physical
Therapist - Discipline: Therapy - Duration: 13 weeks - 40 hours per

Requirements /n - Bachelor's, Master's or Doctoral Degree in
Physical Therapy from accredited schools approved by the APTA -
Active State License is Required to Start the Assignment - BLS
Certification May Be Required from AHA or ARC - Experience in Home
Health is required - Lymphedema certification is a plus /n
Assignment Details /n - Facility Type: Home Health - Shift Name:
Days - Points per visit: 6 points/day (regular visit=1 pt, start of
care=2.5 pts, eval 1.5 pts) - EMR: Homecare Homebase - Mileage
reimbursement: $0.50/mile from first visit to last visit /n
