management across multiple nursing homes, including Ingleside
Communities located in Mount Horeb, WI; Edgerton Care Center,
located in Edgerton, WI; and Montello Care Center, located in

Wound Care Nurse plays a critical role in the assessment, planning,
implementation, and evaluation of patient care related to wound
management. This position requires a compassionate and skilled
nursing professional who is dedicated to improving patient outcomes
through effective wound care practices. The Wound Care Nurse will
report directly to the Director of Nursing and work collaboratively
with healthcare teams to ensure that patients receive comprehensive
care tailored to their individual needs.Responsibilities and
Duties:The ability to develop and facilitate growth within the
Nursing Department relates professionally and positively to
facility and departmental staff and to work cooperatively with
others at all levels.Performs assessments, develops and implements
appropriate plan of care based on assessment and treatment plan
tailored to meet each individual's needs. Evaluates the plan of
care and makes necessary changes as needed.Works collaboratively
with the Director of Nursing, other nurse managers, primary
physicians, nurse practitioners and Medical Director.Provide
primary skin care to residents under the medical direction and
supervision of the residents' attending physicians, medical
director, or the Director of Nursing Services, with an emphasis on
treatment and therapy of skin disorders.Removing dressings,
cleaning wounds, applying medication per physician protocol and
redressing wounds.Delegates and collaborates in providing resident
care according to licensure and skill levels, follows up to ensure
success.Documents according to current standards and established
policies.Ability to work independently with organizational skills,
to prioritize tasks and to exercise judgment consistent with core
values.Ability to model effective communication between residents,
family, staff, and community.Responsible for knowing, implementing
and assure compliance with State, Federal or County regulations,
policies and procedures.Identifies, plans, implements, and
evaluates skin care in residents.Performs an assessment evaluation
using techniques including observation, and inspection.Provides
treatment for skin disorders within an established wound care
protocol.Assists in modifying the treatment regimen to meet the
needs of the residents, in accordance with established medical
practices, the regulatory guidelines and the policies and goals of
this facility.Provides information to residents and caregivers on
prevention techniques and other ways to maintain or improve the
general health and psychosocial status of the residents.Other

assignedQualifications/RequirementsLicensure as an RN in the state
of Wisconsin in good standingWound/Skin Care Certification is
RequiredAt least two-year experience in LTC which includes
extensive wound care knowledge related to treatment and
preventionExcellent communication and organizational skillsStrong
desire to work with the elderlyMust be able to read, write and
communicate in EnglishMust be able to travel as this is a regional
positionQualifications/Requirements PreferredPossess skills in
leadership and communicationCPR certifiedThis position is ideal for
