are seeking a dedicated and compassionate Care Manager to join our
team. This role involves managing a caseload of program members,
collaborating with an RN Care Manager, and providing comprehensive
assessments and care plans. The position is part of a
mission-driven organization focused on helping elderly individuals
and adults with disabilities live independently. Responsibilities
Manage an assigned caseload of program members in collaboration
with an RN Care Manager. Complete comprehensive member assessments,
member-centered care plans, and functional screens. Provide ongoing
care coordination and quality assurance monitoring of services.
Maintain required documentation in adherence to established
standards. Provide direct social work case management services.
Facilitate team management and maintain documentation standards.
Regular local travel within Racine county, requiring use of a
personal vehicle, valid driver's license, and proof of insurance
coverage. Skills & Qualifications 1 to 2 years case management
experience with the elderly and/or developmentally or physically
disabled population preferred. Bachelor’s Degree in a Human
Services or closely related area required. A minimum of three (3)
years’ experience working with seniors or adults with physical
and/or intellectual disabilities may be substituted for a non-human
services degree. Experience with older adults or persons with
physical and/or intellectual disabilities in a social service
capacity preferred. Master’s degree in Social Work preferred
Current Wisconsin Social Work licensure preferred Work Environment
The position is based in Milwaukee with telework currently being
the norm. Work hours are Monday to Friday, 8:00 AM to 4:00 PM,
including travel within Racine county. A valid driver’s license is
required. Job Type & Location This is a Contract to Hire position
based out of Racine, WI. Pay and Benefits The pay range for this
position is $23.00 - $35.00/hr. Eligibility requirements apply to
some benefits and may depend on your job classification and length
